Registration of One-time Donation Receipt Information for Animal Free Initiative
This application form can only be verified by Animal Free Initiative and is used solely for the purpose of registering donation receipt information for year-end tax settlement regarding one-time donations.
※ Donation receipts can be checked on the National Tax Service's Year-End Tax Settlement Simplification Service in early January of every year.
Example: Kim Go-soon / If the depositor's name and the donor's name are different, please be sure to write them separately.
Please write the name of the person to be registered on the National Tax Service's year-end tax simplification website.
